Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans focused on recovery
Provide skilled therapy to improve mobility, strength, balance, and functional independence
Document patient progress and treatment outcomes accurately and timely
Collaborate with interdisciplinary healthcare teams to coordinate optimal care
Educate patients and caregivers on therapy plans and home exercise programs
Adjust treatment plans based on patient response and clinical judgment

Qualifications

Active California Physical Therapist license
Experience in a Skilled Nursing or rehabilitation setting preferred
Strong clinical skills with a focus on patient-centered care
Excellent communication skills to interact with patients, families, and team members
Ability to work independently while contributing effectively as part of a team

Knowledge & Skills

Patient evaluation and assessment
Treatment planning and individualized therapy programs
Mobility, strength, and balance interventions
Skilled documentation and tracking of treatment outcomes
Interdisciplinary collaboration and care coordination
Clear communication and teamwork skills
