Roles & Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans that promote function and independence
Provide therapeutic exercises, gait training, balance training, and mobility programs to improve safety and mobility
Document evaluations, progress notes, and discharge summaries accurately and promptly to support quality care
Collaborate with interdisciplinary care teams to coordinate interventions and optimize patient outcomes
Educate patients and caregivers on treatment plans and safety techniques to encourage long-term success

Qualifications

Active California Physical Therapist (PT) license required
Graduate of an accredited Physical Therapy program
SNF experience preferred and helpful for a smooth transition into this setting
Strong communication and documentation skills to work effectively with clinicians and families
Comfort working in a team-oriented skilled nursing environment and managing varied caseloads
Available for a 13-week contract assignment

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Design and implement therapeutic exercise programs tailored to older adults
Gait training, balance interventions, and mobility program development
Skilled documentation practices and clinical note writing
Patient and caregiver education techniques that enhance adherence and safety
Interdisciplinary collaboration and care coordination in a skilled nursing setting
Strong interpersonal and communication skills
