Roles & Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans that promote function and independence
- Provide therapeutic exercises, gait training, balance training, and mobility programs to improve safety and mobility
- Document evaluations, progress notes, and discharge summaries accurately and promptly to support quality care
- Collaborate with interdisciplinary care teams to coordinate interventions and optimize patient outcomes
- Educate patients and caregivers on treatment plans and safety techniques to encourage long-term success
Qualifications
- Active California Physical Therapist (PT) license required
- Graduate of an accredited Physical Therapy program
- SNF experience preferred and helpful for a smooth transition into this setting
- Strong communication and documentation skills to work effectively with clinicians and families
- Comfort working in a team-oriented skilled nursing environment and managing varied caseloads
- Available for a 13-week contract assignment
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Design and implement therapeutic exercise programs tailored to older adults
- Gait training, balance interventions, and mobility program development
- Skilled documentation practices and clinical note writing
- Patient and caregiver education techniques that enhance adherence and safety
- Interdisciplinary collaboration and care coordination in a skilled nursing setting
- Strong interpersonal and communication skills
