Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans that advance recovery and independence
Provide skilled therapy interventions to improve mobility, strength, balance, and functional outcomes
Document evaluations, treatments, and patient progress accurately and in a timely way
Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes
Ensure compliance with facility policies and state regulations while advocating for patient-centered care

Qualifications

Active Physical Therapist license in the state of California (required)
Skilled Nursing Facility experience preferred but not required
Strong clinical reasoning, communication, and documentation skills
Patient-focused and outcome-driven approach to care

Knowledge & Skills

Proficient in patient evaluation and treatment planning
Patient-centered care and clinical autonomy
Accurate documentation and familiarity with clinical record-keeping
Strong collaboration and communication with interdisciplinary teams
Understanding of SNF workflows and regulatory compliance
