Roles & Responsibilities

• Provide compassionate hands-on care and assistance with activities of daily living to promote resident dignity and comfort
• Assist with bathing, dressing, toileting, feeding and mobility while encouraging independence
• Take and record vital signs and observe residents for changes in condition, reporting concerns promptly to nursing staff
• Follow individualized care plans and document care accurately and timely
• Support infection control practices including proper PPE use and participation in fit testing as required
• Collaborate with nursing and interdisciplinary team members to deliver consistent, resident-centered care
• Contribute to a positive living environment by engaging residents in conversation and respectful interaction
• Participate in required competency testing and maintain current certifications

Qualifications

• Current State CNA certification required; Michigan certification preferred
• Basic Life Support (BLS) certification required
• License and certificate verification required prior to start
• Successful completion of Trinity Specialty Specific Competency Testing as part of onboarding
• Must be able to pass health and compliance requirements: TB screening (2 step PPD or IGRA), Trinity 10 panel drug screen, respirator evaluation and fit test
• Up-to-date immunizations including MMR, Hepatitis B, Varicella, Tdap, COVID-19 and seasonal influenza or documented exemptions
• Ability to work independently in a fast-paced long term care environment and be a dependable team player
• Strong communication skills and a resident-first attitude

Education

• High school diploma or GED preferred
• State Certified Nursing Assistant (CNA) certification required

Knowledge & Skills

• Skilled in personal care and activities of daily living assistance
• Knowledge of infection prevention and proper use of PPE
• Ability to recognize and report changes in resident condition
• Comfortable with basic charting and documentation practices
• Good interpersonal skills for working with residents, families and care teams
• Safe transfer and mobility assistance techniques
• Time management and prioritization during busy shifts
• Commitment to resident dignity, privacy and person-centered care
