Roles & Responsibilities
- Provide compassionate bedside care to residents, promoting dignity and independence
- Administer medications and treatments safely within LPN scope of practice
- Perform and document routine assessments, vital signs, and care needs
- Collaborate with RNs, interdisciplinary team members, and families to deliver coordinated care
- Maintain accurate, timely clinical documentation in the patient record
- Follow infection control procedures and facility safety protocols at all times
- Assist with admissions, transfers, and discharges to ensure smooth transitions
- Participate in competency testing and continuing education to maintain high standards
Qualifications
- Current LPN license in Michigan or compact-equivalent eligibility
- Current BLS (Basic Life Support) certification
- Successful License/Certificate Verification prior to start
- Completion of Trinity Specialty Specific Competency Testing as required
- Ability to pass required health screenings: Fit Test, 2-step PPD or IGRA, required immunizations
- Clean Trinity 10 Panel Drug Screen and satisfactory physical/health statement
- Current Tdap, MMR, Hepatitis B, Varicella, and COVID-19 vaccination (per facility policy)
- Strong communication skills and a caring, patient-focused attitude
Education
- Graduate of an accredited Practical Nursing / LPN program; LPN diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and dressing changes appropriate to LPN scope
- Accurate vital sign measurement and recognition of clinical changes
- Electronic health record navigation and timely charting
- Infection prevention and control practices
- Time management, prioritization, and teamwork in a fast-paced environment
- Clear communication with residents, families, and clinical staff
