Roles & Responsibilities

• Provide compassionate direct nursing care to residents as an LPN, promoting dignity and comfort
• Administer medications and treatments according to provider orders and facility policies
• Perform thorough resident assessments and document care clearly and accurately
• Collaborate with RNs and the interdisciplinary team to implement individualized care plans
• Support infection prevention measures, including Fit Test and respirator protocols
• Participate in Trinity Specialty Specific Competency Testing to maintain high-quality care
• Assist with transitions of care and communicate proactively with families and providers

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing
• BLS certification (required)
• Successful License/Certificate Verification as part of onboarding
• Completion of Trinity Specialty Specific Competency Testing as required
• Current immunizations and health screenings: Fit Test, 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, and Trinity 10-panel drug screen
• Ability to pass Trinity respirator evaluation and color vision screening

Education

• Graduate of an accredited practical nursing program (LPN/LVN) or equivalent

Knowledge & Skills

• Solid knowledge of long-term care nursing and resident-centered practices
• Medication administration and documentation skills
• Infection control and safety-conscious work habits
• Good communication and teamwork with clinical colleagues and families
• Ability to follow protocols for health screenings and competency testing
