Location: Pensacola, FL
Contract Type: Travel Contract
Shift Type: Per diem — varied shifts (day, evening and night) based on assignment from the PD Pool; schedule as-needed and may include weekends

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to PACE participants in accordance with plan of care
- Administer medications and document treatments accurately and timely
- Perform assessments and report changes in participant condition to the care team
- Follow infection control protocols and maintain safe clinical practices
- Complete Trinity Skills Checklist and required documentation to ensure quality and compliance
- Collaborate with interdisciplinary team members to support participant wellbeing and independence
Qualifications
- Current Licensed Practical Nurse (LPN) license (Florida) — license verification required
- BLS certification (current)
- Completion of Trinity Skills Checklist as part of onboarding
- Up-to-date immunizations and health screening per policy (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza, 2-step PPD or IGRA)
- Successful Trinity 10 Panel Drug Screen and required health/physical documentation
- Reliable, professional demeanor with strong communication and documentation skills
Education
- Graduation from an accredited Practical Nursing program (LPN) or equivalent completion of state-approved LPN curriculum
Knowledge & Skills
- Fundamentals of adult nursing care, medication administration and wound care
- Infection prevention and safe clinical practice
- Accurate charting and electronic documentation skills
- Ability to follow care plans and report clinical changes effectively
- Patient-centered care and strong interpersonal skills for working with older adults
- Comfortable working independently on per diem assignments and collaborating with an interdisciplinary team
