Roles & Responsibilities
- Provide compassionate, professional nursing care to residents in the Trinity Health LIFE program
- Administer medications and treatments accurately and safely according to policy
- Monitor and document patient conditions, reporting changes to the care team promptly
- Assist with care planning and coordinate with interdisciplinary team members
- Perform routine procedures and basic wound care per facility protocols
- Maintain clear, timely charting in the electronic medical record
- Support resident dignity and independence through patient-centered care
- Adhere to infection control, safety standards, and facility policies at all times
Qualifications
- Current LPN license valid in New Jersey
- Completion of an accredited LPN program
- Current BLS (Basic Life Support) certification preferred or required upon hire
- Must meet Trinity Health health requirements: MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement, COVID-19 vaccine, Influenza vaccine
- Successful completion of Trinity 10-panel drug screen and required background checks
- Strong communication skills and the ability to collaborate with a multidisciplinary team
- Prior experience in long-term care, community health, or similar settings is a plus
Education
- Completion of an accredited Licensed Practical Nurse (LPN) program; high school diploma or equivalent required
Knowledge & Skills
- Proficient in medication administration and basic nursing procedures
- Familiarity with resident assessment and documentation practices
- Ability to follow care plans and implement physician/nurse directives
- Good clinical judgment, prioritization, and time management
- Comfortable using electronic medical records and standard clinical technology
- Compassionate bedside manner and ability to engage with older adults
