Roles & Responsibilities

• Deliver compassionate direct patient care and assist with activities of daily living
• Perform vital signs, intake/output tracking, and basic observations accurately
• Assist patients with transfers, ambulation, bathing, grooming, and toileting
• Document care and patient status clearly and promptly in the chart
• Communicate changes in patient condition to RNs and the care team
• Practice and promote infection control and safety procedures at all times
• Support a positive patient experience through dignity, respect, and empathy
• Participate in shift handoffs and collaborate with the interdisciplinary team

Qualifications

• Current Licensed Nursing Assistant (LNA) credential in New Hampshire or eligible to practice in NH/VT
• Completion of a state-approved LNA program
• CPR/BLS certification preferred or ability to obtain upon hire
• Ability to work night shifts and weekend schedule (Fri, Sat, Sun)
• Willingness to complete required immunizations, background check, and drug screening

Education

• High school diploma or equivalent and completion of a state-approved LNA program

Knowledge & Skills

• Strong fundamentals in patient care and ADLs
• Accurate vital signs measurement and basic clinical observation
• Safe patient transfer and mobility assistance techniques
• Basic charting and documentation skills; comfort with electronic records
• Infection control, safety awareness, and attention to detail
• Compassionate communication and teamwork in a fast-paced environment
• Time management and the ability to prioritize patient needs
