Roles & Responsibilities
- Provide direct physical therapy treatment under the direction of a licensed physical therapist
- Implement and progress therapeutic exercise and mobility plans to improve patient function
- Document patient evaluations, treatment notes, and progress accurately and timely
- Collaborate with nursing and the interdisciplinary team to support patient goals
- Educate patients and families on home exercise programs and safety strategies
- Follow the physician's plan of care and adhere to the state Physical Therapy Practice Act and facility policies
Qualifications
- Licensed Physical Therapist Assistant (PTA) in New Mexico or eligible for licensure
- BLS/CPR certification required or obtained upon hire
- Comfortable working in a skilled nursing facility setting
- All experience levels considered, including new graduates
- Ability to pass background check, drug screen, and required immunizations
Knowledge & Skills
- Therapeutic exercise and mobility training for SNF patients
- Safe transfer techniques and fall prevention strategies
- Skilled documentation and familiarity with EMR/charting
- Good clinical judgment and ability to follow PT plan of care
- Strong communication and teamwork skills with patients and staff
