Roles & Responsibilities

• Provide direct physical therapy treatment under the direction of a licensed physical therapist
• Implement and progress therapeutic exercise and mobility plans to improve patient function
• Document patient evaluations, treatment notes, and progress accurately and timely
• Collaborate with nursing and the interdisciplinary team to support patient goals
• Educate patients and families on home exercise programs and safety strategies
• Follow the physician's plan of care and adhere to the state Physical Therapy Practice Act and facility policies

Qualifications

• Licensed Physical Therapist Assistant (PTA) in New Mexico or eligible for licensure
• BLS/CPR certification required or obtained upon hire
• Comfortable working in a skilled nursing facility setting
• All experience levels considered, including new graduates
• Ability to pass background check, drug screen, and required immunizations

Knowledge & Skills

• Therapeutic exercise and mobility training for SNF patients
• Safe transfer techniques and fall prevention strategies
• Skilled documentation and familiarity with EMR/charting
• Good clinical judgment and ability to follow PT plan of care
• Strong communication and teamwork skills with patients and staff
