Roles & Responsibilities

• Perform hands-on physical therapy assessments and create individualized treatment plans to help patients regain function
• Deliver evidence-based interventions, including therapeutic exercise and manual therapy, to produce measurable outcomes
• Monitor patient progress and update goals to ensure continuous improvement and meaningful results
• Document clinical progress accurately and promptly in the electronic health record to maintain high-quality care
• Educate patients and families to promote long-term self-management and independence
• Collaborate with an interdisciplinary team to deliver coordinated, patient-centered care
• Manage caseload effectively and prioritize patient needs to maximize outcomes
• Foster a positive clinic culture that supports colleagues and enhances the patient experience

Qualifications

• Licensed Physical Therapist in Minnesota or ability to obtain Minnesota licensure promptly
• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable where applicable
• Current BLS/CPR certification or willingness to obtain prior to start
• Strong clinical assessment and manual therapy skills
• Excellent communication and teamwork skills with a patient-centered approach
• Previous outpatient experience preferred; new graduates with strong clinical skills are welcome

Education

• Doctor of Physical Therapy (DPT) preferred
• Master’s or Bachelor’s in Physical Therapy acceptable where applicable

Knowledge & Skills

• Musculoskeletal evaluation and tailored treatment planning
• Therapeutic exercise prescription with safe progression
• Strong manual therapy and hands-on treatment techniques
• Familiarity with documentation standards and electronic health records
• Good time management and caseload prioritization
• Ability to engage patients and educate families effectively
• Comfort working in a community clinic setting and collaborating with a care team
