    Roles & Responsibilities
- Perform comprehensive OASIS and physical therapy assessments to set meaningful, measurable goals
- Develop individualized plans of care that promote independence, safety, and lasting progress
- Deliver skilled physical therapy interventions and teach patients and caregivers practical strategies
- Document clinical progress and OASIS time points accurately and promptly to support quality care and billing
- Coordinate care with physicians, nurses, caregivers, and the rehab team to ensure seamless continuity
- Optimize visit schedules to boost productivity and achieve better patient outcomes
- Supervise PTAs, students, and home care aides while maintaining regulatory and clinical compliance
- Lead and participate in quality improvement initiatives and peer reviews to elevate standards of care
- Engage in community outreach and marketing to grow services and promote rehab programs
Qualifications
- Current and unrestricted Vermont Physical Therapist license required
- Doctor of Physical Therapy (DPT) preferred; Master of Physical Therapy acceptable
- BLS/CPR certification or willingness to obtain quickly
- Valid driver license, reliable transportation, and comfort traveling between patient homes
- Ability to pass background checks and complete credentialing
- Prior home health or community-based physical therapy experience preferred
- Comfort supervising PTAs and students and collaborating with interdisciplinary teams
Education
- Doctor of Physical Therapy (DPT) preferred
- Master of Physical Therapy acceptable
- State Physical Therapist licensure required
Knowledge & Skills
- Excellent communication, time management, and care coordination skills
- Strong patient assessment skills and plan of care development
- Skilled in therapeutic interventions and patient and caregiver education
- Proficient with clinical documentation systems; McKesson experience is a plus
- Knowledge of OASIS data collection and home health quality improvement processes
- Understanding of billing and charging documentation and insurance coverage considerations
- Familiarity with Federal and State regulations and APTA guidelines
- Strong clinical record keeping to support quality and reimbursement
- Experience supervising clinical staff and promoting clinical best practices
