Roles & Responsibilities
- Assess patients and caregivers using the nursing process to identify needs and set measurable goals
- Complete timely, accurate OASIS documentation that meets agency, state, and federal standards
- Develop and communicate individualized, evidence-based plans of care to guide treatment and recovery
- Manage an assigned caseload and visit schedule to meet productivity and quality expectations
- Implement medical plans of care and nursing interventions to achieve safe, effective outcomes
- Perform advanced home health skills such as IV therapy, wound VAC management, and complex wound care
- Coordinate care with physicians, therapists, social workers, vendors, and families to ensure smooth transitions
- Educate patients and caregivers on medications, treatments, and discharge plans with compassion and clarity
- Participate in peer review audits and quality improvement activities to elevate standards of care
Qualifications
- Active RN license in Vermont (compact or state-issued) required
- Associate Degree in Nursing (ADN) required; BSN preferred
- BLS required; ACLS or other specialty certifications preferred
- Home health, case management, or OASIS documentation experience recommended; 1+ years preferred
- Valid driver’s license and reliable transportation for travel to patient homes
- Comfortable delegating to and collaborating with LPNs and home care aides
- Strong clinical judgment, excellent documentation skills, and solid time management
- Able to work independently in the field while staying connected to a supportive team
Education
- Associate Degree in Nursing (ADN) - required
- Bachelor of Science in Nursing (BSN) - preferred
Knowledge & Skills
- OASIS documentation standards and timely completion expectations
- Care planning and evidence-based nursing interventions
- Infection control principles and wound care best practices
- IV therapy, wound VAC management, and other high-tech home health skills
- McKesson electronic processes for visit documentation and follow-up
- Patient and caregiver education delivered with clarity and compassion
- Interdisciplinary communication and case coordination skills
- Quality improvement and participation in peer review activities
