Roles & Responsibilities
- Admit patients to hospice services and build compassionate, trust-based relationships with patients and families
- Perform comprehensive clinical and psychosocial assessments to guide individualized care planning
- Develop and implement personalized care plans that honor patient wishes and clinical needs
- Document clinical observations and interventions accurately and in a timely manner
- Coordinate care across multidisciplinary teams to ensure seamless, high-quality services
- Participate in quality assessment and performance improvement activities to enhance patient outcomes
- Conduct follow-up visits, reassess needs, and adjust plans of care as conditions change
- Advocate for patients by communicating clearly with caregivers and other providers
Qualifications
- Active, current RN license in California (required)
- Minimum 1 year of hospice nursing experience (required)
- BLS (Basic Life Support) certification (required)
- Hospice or palliative care certifications are a plus
- Valid driver license and reliable transportation for local travel within the territory
- Strong clinical judgment and ability to work independently in the field
- Excellent communication skills for family-centered care and multidisciplinary coordination
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
- Active RN license in California
- BLS (Basic Life Support) certification
Knowledge & Skills
- Skilled in patient assessment and comprehensive care planning
- Comfortable with accurate clinical documentation and timely charting
- Experienced coordinating care across multidisciplinary teams
- Able to manage caseloads, prioritize visits, and perform effective follow up
- Professionalism and sensitivity in end-of-life conversations and family support
- Familiarity with quality assessment and performance improvement processes
- Strong clinical decision making for independent field practice
- Clear, compassionate communication with patients, families, and providers
