    Roles & Responsibilities
- Provide compassionate, hands-on nursing care that preserves resident dignity and independence
- Administer medications and treatments accurately to keep residents safe and comfortable
- Monitor resident conditions and report meaningful changes promptly to the care team
- Document thorough assessments and care in the medical record to support quality outcomes
- Collaborate with nurses, therapists, and facility staff to follow individualized care plans
- Assist residents with activities of daily living while promoting mobility and autonomy
- Participate in quality and compliance initiatives to help maintain high standards of care
Qualifications
- Current, unrestricted Colorado LPN license required
- Graduate of an accredited Practical Nursing (LPN) program
- Current BLS/CPR certification or willingness to obtain one
- Ability to pass a background check and meet employer screening requirements
- Long-term care experience preferred
- MDS experience is a plus
- Strong communication, empathy, and a team-oriented attitude
Education
- Graduate of an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Knowledge of long-term care nursing practices and regulatory requirements
- Medication administration and basic clinical assessment skills
- Familiarity with MDS processes and electronic medical records
- Strong communication, empathy, and teamwork abilities
- Good organizational skills and the ability to prioritize resident needs
- Ability to document clear, accurate clinical notes and assessments
