Roles & Responsibilities
- Evaluate patients across the lifespan to measure mobility, set meaningful goals, and track progress
- Develop individualized treatment plans that promote independence and deliver measurable outcomes
- Provide hands-on therapy and design home exercise programs to support long-term recovery
- Perform wheelchair fittings, lymphedema management, and specialty interventions when indicated
- Collaborate with physicians, nurses, and the rehab team to coordinate patient-centered care
- Document patient progress accurately and promptly in the electronic medical record
- Educate patients and families on strategies to maximize function and safety
Qualifications
- Current, unencumbered Oregon Physical Therapy license (required)
- Healthcare Provider-level BLS certification (required)
- Comfortable traveling to multiple points of care within the service area
- Experience or a strong interest treating patients from pediatrics through geriatrics
- Familiarity or interest in neurological care, women’s health, and lymphedema treatments
- Strong communication, patient education, and accurate documentation skills
- Proven ability to collaborate effectively with interdisciplinary healthcare teams
Education
- Doctor of Physical Therapy (DPT) preferred
- Master's in Physical Therapy acceptable where applicable
- State Physical Therapy license (Oregon) required
Knowledge & Skills
- Patient evaluation and outcome measurement to demonstrate improvement
- Treatment planning and progression using manual therapy and therapeutic exercise
- Wheelchair fitting and assistive device expertise
- Understanding of neurological, pediatric, geriatric, women’s health, and lymphedema care
- Effective patient and family communication and education techniques
- Electronic medical record documentation and timely charting
