Roles & Responsibilities

• Perform timely Start of Care visits so patients begin therapy when they need it most
• Conduct thorough clinical assessments and develop individualized, effective care plans
• Deliver clear patient education that boosts independence and improves outcomes
• Supervise and mentor Physical Therapy Assistants to maintain high-quality care
• Complete Start of Care reports and required clinical documentation within established timelines
• Coordinate with intake teams, supervisors, and office staff to ensure smooth transitions of care
• Manage an assigned Solano County territory efficiently, balancing caseload, travel, and productivity
• Meet weekly productivity expectations and optimize visit mixes to maximize patient benefit

Qualifications

• Active California Physical Therapist license required
• Minimum 1 year of home health experience
• DPT or equivalent degree that led to California PT licensure
• Experience supervising PTAs or clinical staff strongly preferred
• Valid driver’s license and reliable transportation for home visits; company car option available
• Comfortable using mobile devices and company documentation tools
• Strong documentation and communication skills with ability to meet next-day and 48-hour timelines

Education

• DPT or equivalent degree leading to California Physical Therapist licensure

Knowledge & Skills

• Familiarity with home health workflows and OASIS-related visit types
• Experience using the Kinnser documentation system
• Ability to convert productivity points to hours and manage visit mixes
• Strong clinical assessment, evaluation, and patient education skills
• Clear professional communication with patients, families, and interdisciplinary teams
• Time management and territory planning for efficient travel between visits
• Accurate, timely documentation to meet next-day and 48-hour requirements
• Comfortable with mobile documentation and company-provided tech tools
