Roles & Responsibilities

• Assess patients to determine physical, mental, and psychosocial needs and develop person-centered care plans
• Implement and evaluate individualized patient care in partnership with the multidisciplinary team
• Educate patients and families clearly and compassionately to support informed decisions
• Quickly recognize changes in patient condition using strong clinical judgment and escalate appropriately
• Maintain accurate medical records and fulfill Core Measure responsibilities tied to quality care
• Manage conflicts professionally and foster a positive, team-focused workplace
• Participate in continuing education to expand clinical expertise and keep certifications current

Qualifications

• Current Illinois Registered Nurse (RN) license required
• Current CPR/BLS card required
• ACLS certification required
• Comfortable working four 10-hour day shifts and participating in an on-call rotation
• Experience in progressive care, telemetry, or critical care preferred
• Strong clinical judgment, excellent assessment skills, and proven teamwork ability

Education

• Graduate of an accredited nursing program (ADN or BSN); BSN preferred

Knowledge & Skills

• Patient assessment and individualized care planning in acute and progressive settings
• Clinical judgment and rapid recognition of changes in patient condition
• Accurate documentation and medical records management
• Clear, compassionate patient and family education
• Familiarity with Core Measures, quality, and safety standards
• Proficiency in BLS and ACLS
• Telemetry and progressive care monitoring experience (preferred)
• Effective teamwork and professional conflict management
