Bill Rate: $10,000 sign-on; $10,000 relocation bonus

Roles & Responsibilities
- Conduct clinical screening, assessment, diagnosis, and treatment planning for patients from birth through elder
- Deliver brief therapy and continuing care interventions tailored to patient and family needs
- Lead and facilitate discharge planning and patient care conferences, including placements to ALF or transitional care
- Coordinate care with hospital teams and community partners to ensure smooth transitions and optimal outcomes
- Provide client advocacy and support for complex matters such as Power of Attorney when relevant
- Maintain accurate, timely, and compliant documentation for screenings, assessments, treatment plans, progress notes, and required reports
- Support clinical needs related to chemical dependency and mental health and make appropriate referrals
- Participate in staff trainings, quality management activities, and provider enrollment/billing tasks as assigned
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
- Licensed (or in process of licensure) as LCSW, LPC, or LMFT
- Valid State of Alaska driver’s license or ability to obtain within 6 months
- Must be 21 years of age
- Clinical competency assessment required within 3 months of hire and every three years (SEARHC Clinical Competency Assessment)
- Ability to complete timely and compliant documentation in accordance with state, federal, accreditation, and hospital billing standards
Education
- Master's Degree in Social Work, Psychology, Counseling, or related discipline
Knowledge & Skills
- Discharge planning and care coordination
- Clinical assessment, screening, and diagnosis
- Brief therapy and treatment planning
- Case management and client advocacy
- Crisis intervention and interviewing techniques
- Familiarity with accreditation and documentation regulations
- Experience working with tribal health and cultural wellness principles
- Ability to perform provider enrollment and basic billing/provider support tasks
