Roles & Responsibilities

• Conduct thorough evaluations to assess resident communication and swallowing needs
• Provide daily treatments that are patient-centered and evidence-based
• Manage timely and safe discharges with appropriate follow-up recommendations
• Collaborate closely with the therapy team to deliver coordinated care
• Meet and track productivity expectations (90% productivity requirement)
• Follow CRG policies and procedures to maintain high clinical and compliance standards
• Maintain professional appearance (black scrubs) and be present on-site to ensure guaranteed hours

Qualifications

• 1-3 years of relevant experience (listed as 1-3 years)
• Prior SNF experience is strongly preferred
• Ability to be on-site and meet the facility's productivity and presence requirements
• Comfort following CRG policies and facility dress code (black scrubs)

Knowledge & Skills

• Skilled in evaluations, daily treatment planning, and discharge planning
• Experience working in a skilled nursing facility (SNF) environment
• Strong documentation and time-management skills to meet productivity targets
• Ability to collaborate with a multidisciplinary therapy team
• Familiarity with facility policies and procedures and willingness to follow them
