Roles & Responsibilities

• Conduct thorough speech-language evaluations to identify patient needs
• Deliver daily, evidence-based treatments tailored to each resident
• Manage timely and accurate discharge planning and documentation
• Collaborate closely with the therapy team and nursing staff to coordinate care
• Maintain productivity targets and documentation to meet facility standards
• Follow Continuum Rehabilitation Group (CRG) policies and procedures
• Represent the facility professionally (dress code: black scrubs) and be present on-site to receive guaranteed hours

Qualifications

• Active SLP credential or state license (must meet state requirements)
• 1-3 years of SLP experience (minimum 1 year preferred by facility)
• Prior SNF experience strongly preferred
• Comfortable with productivity expectations (90%) and facility documentation
• Ability to work on-site and engage with interdisciplinary team members

Knowledge & Skills

• Skilled in conducting speech-language evaluations and treatment planning
• Experience with daily therapeutic interventions and discharge processes
• Strong documentation and clinical record-keeping skills
• Ability to work collaboratively within a multidisciplinary SNF team
• Familiarity with SNF workflows, productivity standards, and facility policies
