Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families using evidence-based teaching strategies
• Assess patients using the nursing process and collaborate with interdisciplinary team members to develop individualized care plans
• Administer prescribed medications and treatments safely and in accordance with policies, standards of care, and state practice acts
• Document care and education clearly and accurately according to clinical standards
• Serve as a clinical resource for patient education materials and processes and support continuity of care across units
• Participate in performance improvement initiatives and shared governance to enhance patient outcomes
• Escalate safety or clinical concerns through the appropriate chain of command to protect patient well-being

Qualifications

• Licensed as a Registered Nurse (RN) in Illinois and Wisconsin required
• Graduate of an accredited nursing program; BSN preferred
• CDCES certification required (obtain within 90 days as noted)
• Approved CPR certification required within 90 days
• Experience with patient assessment, medication administration, documentation, and care plan development
• Comfortable collaborating with interdisciplinary teams and participating in education and performance improvement activities
• Willingness to float to other units and work occasional weekends/holidays or on-call as needed

Education

• Graduate of an accredited nursing program (BSN preferred)
