Roles & Responsibilities

• Evaluate patients and develop individualized, evidence-based treatment plans
• Provide hands-on physical therapy services across inpatient, swing bed, outpatient and skilled nursing settings
• Document patient progress accurately and timely in the EMR
• Collaborate with physicians, nurses and interdisciplinary team members to coordinate care
• Apply strong clinical reasoning to manage diverse diagnoses and patient populations
• Educate patients and families on home programs and safe functional mobility
• Ensure compliance with California and federal PT regulations and physician orders
• Participate in professional development and continuous quality improvement activities

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• 1+ year of experience in acute care, swing bed/skilled nursing, or outpatient settings
• Strong documentation skills and EMR proficiency
• Excellent communication, clinical reasoning, and teamwork abilities

Education

• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and treatment planning
• Providing physical therapy services across care settings
• Accurate documentation and EMR proficiency
• Clinical reasoning and problem-solving
• Ability to manage a diverse patient population
• Strong attention to detail and effective communication
• Basic computer proficiency
