Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to maximize functional outcomes
• Develop and implement individualized treatment plans tailored to each resident's goals
• Collaborate with an interdisciplinary team to coordinate seamless, patient-centered care
• Document clinical findings and progress accurately and in a timely manner according to facility and regulatory standards
• Participate in care conferences and contribute to ongoing quality improvement efforts
• Support and mentor therapy aides/assistants and help foster a learning environment

Qualifications

• Active California Physical Therapist license (required)
• Skilled nursing or post-acute experience preferred
• Strong clinical and communication skills to connect with residents and the care team
• Dependable, flexible, and team-oriented with a commitment to compassionate care
