Roles & Responsibilities

• Perform thorough patient evaluations to establish baseline function and goals
• Design and implement individualized treatment plans that promote mobility and independence
• Deliver evidence-based physical therapy interventions and therapeutic modalities
• Educate patients and families on safe movement, therapeutic exercises, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and care teams to coordinate and optimize care
• Monitor patient progress, adjust plans as needed, and document outcomes accurately
• Ensure care plans meet regulatory standards and facility policies

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of California (must be active and in good standing)
• Experience in a skilled nursing facility (SNF) or similar setting preferred
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently while contributing to an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

Knowledge & Skills

• Patient evaluation and assessment techniques
• Treatment planning and implementation
• Physical therapy interventions and therapeutic modalities
• Use of adaptive equipment and safe mobility strategies
• Accurate clinical documentation and progress tracking
• Care coordination and interdisciplinary collaboration
• Understanding of regulatory compliance and best practices in rehab
