Roles & Responsibilities

• Evaluate patients’ mobility, strength and functional needs and develop personalized treatment plans
• Provide skilled therapeutic interventions to improve movement, reduce pain and restore independence
• Document patient progress and outcomes clearly and timely in the electronic health record
• Collaborate with nurses, physicians and rehab team members to coordinate care and discharge planning
• Educate patients and families on home exercise programs and injury prevention strategies
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Valid Physical Therapist license to practice in California (required)
• Current BLS/CPR certification
• Experience in orthopedic, outpatient, or post-acute rehab settings is preferred
• Strong clinical assessment, manual therapy and therapeutic exercise skills
• Ability to complete facility background check and required credentialing in a timely manner
• Excellent communication skills and a collaborative approach to patient-centered care

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy

Knowledge & Skills

• Thorough knowledge of physical therapy evaluation and treatment techniques
• Skilled in therapeutic exercise, gait training, manual therapy and mobility aids
• Proficient documentation and use of electronic health records
• Understanding of discharge planning, safety assessment and patient education best practices
• Ability to work within an interdisciplinary team and adapt to varied patient populations
