Roles & Responsibilities

• Perform comprehensive OASIS and physical therapy assessments to evaluate patient needs and risks
• Develop individualized plans of care with clear, measurable patient goals
• Provide skilled physical therapy treatments using evidence-based therapeutic techniques
• Monitor patient progress each visit and update plans to promote functional independence
• Coordinate care with physicians, nurses, caregivers and rehab team members to ensure seamless care
• Supervise and direct PTAs, PT students and home care aides in line with regulations and APTA guidelines
• Complete accurate clinical documentation, OASIS time points and required agency/state/federal forms on time
• Use McKesson and agency processes to manage visits, follow-up notes and visit completion
• Deliver wound care and staple/suture removal as clinically indicated
• Participate in QI activities, peer review audits and in-service training to support quality outcomes

Qualifications

• Licensed Physical Therapist (current, per state regulations) — home health experience is a strong match
• Proven experience with OASIS assessments and timely documentation
• Familiarity with McKesson workflows and agency visit management
• Comfort supervising PTAs and PT students and providing constructive clinical feedback
• Skilled in wound care and staple/suture removal as part of home health practice
• Working knowledge of APTA guidelines, Medicare forms (HHABN/Notice of Non-Coverage) and federal/state regulations
• Strong care coordination, documentation and patient education abilities
