Roles & Responsibilities

• Assess patients to identify functional deficits and develop personalized occupational therapy plans
• Design and implement evidence-based rehabilitation programs focused on returning patients to safe, meaningful daily activities
• Provide skilled therapeutic interventions and adaptive equipment recommendations to maximize independence
• Collaborate with nursing, PT, SLP and other team members to coordinate comprehensive care
• Document treatment progress, outcomes and discharge planning in a timely, compliant manner
• Educate patients and families on home programs, safety strategies and community resources
• Monitor patient response and modify plans to ensure measurable progress toward goals
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Valid Occupational Therapist license in Michigan or eligibility to obtain Michigan licensure
• Degree from an accredited Occupational Therapy program (MOT, OTD or equivalent)
• Experience in rehabilitation and skilled nursing facility settings preferred
• Current BLS/CPR certification or willingness to obtain upon hire
• Strong documentation skills and comfort with electronic medical records
• Excellent interpersonal and communication skills for patient and team interactions
• Reliable transportation and ability to work on-site at the facility

Education

• Entry-level degree in Occupational Therapy from an accredited program (MOT or OTD preferred)

Knowledge & Skills

• Skilled in patient evaluation, standardized assessments and goal writing
• Knowledge of ADL/IADL retraining, cognitive strategies and adaptive equipment
• Familiarity with safety assessments, fall prevention and discharge planning
• Ability to educate patients and caregivers in clear, compassionate language
• Strong teamwork and communication skills within interdisciplinary rehab teams
• Competence with clinical documentation and basic workplace technology
