Roles & Responsibilities

• Provide skilled, patient-centered physical therapy in home health settings to improve mobility and independence
• Assess functional status, perform mobility and safety evaluations, and develop individualized treatment plans
• Implement therapeutic interventions and monitor patient progress toward goals
• Document timely, accurate patient records (Epic preferred) and complete Trinity skills checklists and competency testing
• Collaborate with nurses, case managers and caregivers to coordinate care and transitions
• Educate patients and families on home exercise programs, fall prevention, and safe equipment use
• Maintain compliance with licensing, certification, and infection control requirements

Qualifications

• Current, unrestricted Physical Therapy license for Iowa (license verification required)
• BLS certification (required)
• Home health experience preferred; comfortable working independently in patient homes
• Epic experience preferred but not required
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass required health screenings (Fit Test, drug screen, immunizations, PPD/IGRA, etc.)

Knowledge & Skills

• Strong assessment and clinical reasoning skills for home health patient populations
• Proficiency in therapeutic exercise, gait training, transfers and fall-risk mitigation
• Knowledge of durable medical equipment and safe home mobility solutions
• Effective patient and caregiver education skills
• Clear, timely documentation and charting (Epic preferred)
• Excellent time management and route planning for home visits
• Collaborative communication with interdisciplinary teams
