Roles & Responsibilities

Provide outpatient-focused primary care to patients across the lifespan
Deliver patient-centered medicine and build long-term relationships with patients and families
Collaborate with experienced clinical and administrative staff as part of a team
Manage a panel of patients with continuity and attention to preventive care
Participate in quality improvement efforts to impact community health
Maintain required credentials and participate in necessary trainings and CME

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
Passion for community-focused, rural healthcare and patient-centered care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Outpatient primary care
Patient-centered medicine
Care across the lifespan
Rural and community healthcare experience or interest
Collaborative care team practice
