Roles & Responsibilities

Conduct comprehensive psychosocial assessments to understand patient needs across the lifespan
Develop individualized, evidence-based treatment plans that address depression, anxiety, trauma, substance use, and adjustment disorders
Deliver evidence-based therapeutic interventions with clinical skill and compassion
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to ensure coordinated, whole-person care
Provide care coordination and referrals for community resources and follow-up services
Maintain timely, accurate clinical documentation to support quality care and continuity

Qualifications

Master's degree in Social Work from an accredited program (required)
Active LISW license in Iowa (required)
Minimum of 2 years clinical experience preferred
Experience working in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and clinical documentation skills

Education
