Roles & Responsibilities

• Perform thorough patient evaluations to assess physical abilities and rehabilitation needs
• Design and implement individualized treatment plans that promote mobility and independence
• Provide hands-on physical therapy interventions and therapeutic modalities to improve strength and function
• Educate patients and families on exercises, safe movement techniques, and home modifications
• Collaborate closely with occupational therapists, nurses, physicians, and the care team to coordinate care
• Monitor patient progress and adjust plans to ensure optimal outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and treatment plans in line with facility policies
• Ensure care plans comply with regulatory requirements and best practices in physical therapy

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license (licensure in California or as required by the facility)
• Experience in a skilled nursing facility or similar post-acute setting preferred
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently and as part of an interdisciplinary team
• Compassionate, patient-focused approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

Knowledge & Skills

• Proficient in physical therapy techniques and therapeutic modalities
• Skilled in functional assessment and treatment planning
• Experience with adaptive equipment and mobility improvement strategies
• Thorough documentation skills and familiarity with regulatory standards
• Ability to educate patients and families in clear, supportive ways
• Comfortable collaborating across disciplines to optimize patient outcomes
