Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families, empowering self-management and healthy choices
• Develop, initiate and execute individualized patient care plans in collaboration with physicians and the multidisciplinary team
• Administer prescribed medications and treatments safely and according to policy and state nursing practice acts
• Accurately document patient care and education in clinical records following standards of care
• Act as a clinical resource for staff on patient education materials, program development and diabetes-related issues
• Participate in performance improvement activities and shared governance to improve patient outcomes
• Utilize critical thinking to identify safety concerns and escalate appropriately to protect patients
• Float to various units as needed, offering clinical expertise to support the patient's longitudinal plan of care

Qualifications

• Licensed as an RN in Illinois and Wisconsin (required)
• Graduate of an accredited nursing program (BSN preferred)
• CDCES (Certified Diabetes Care and Education Specialist) certification required
• Approved CPR certification required within 90 days of hire
• Experience in diabetes education, patient instruction and clinical documentation
• Ability to collaborate in an interdisciplinary team and participate in shared governance
• Willingness to float to various units and work occasional weekends/holidays or on-call as needed

Education

• Graduate of an accredited nursing program; BSN preferred
