Roles & Responsibilities

• Assess patients in their homes and develop individualized treatment plans that improve function and independence
• Provide skilled physical therapy interventions focused on mobility, strength, balance and fall prevention
• Document clinical care accurately and efficiently using HCHB
• Coordinate care with nurses, aides and family members to ensure a cohesive plan for the patient
• Educate patients and caregivers on home exercise programs and safe home modifications
• Manage a focused caseload (typically 5–6 patient visits per day) while maintaining high-quality documentation and outcomes
• Communicate clearly with the interdisciplinary team and refer as needed to optimize patient outcomes

Qualifications

• Minimum 2 years of experience, with home health experience required for therapy positions
• Active Physical Therapist licensure in the state of practice (or eligibility to obtain)
• Comfortable documenting in HCHB (HomeCare HomeBase) or similar electronic documentation systems
• Strong clinical assessment, manual therapy and therapeutic exercise skills
• Reliable vehicle and valid driver’s license for patient visits in the community
• Excellent communication skills and ability to collaborate with a multidisciplinary team

Education

• Degree in Physical Therapy (DPT or Master's in PT preferred)

• Home health clinical best practices and home safety assessments
• Caseload management and efficient visit planning
• Skilled documentation in HCHB and outcomes-driven charting
• Patient and caregiver education techniques
• Ability to prioritize visits and adapt plans to patient home environments
• Professional bedside manner and strong interdisciplinary communication
