Roles & Responsibilities

• Evaluate patients and perform thorough assessments to identify functional needs
• Develop and implement individualized treatment plans to improve mobility and independence
• Provide both outpatient and inpatient therapy coverage with professional clinical judgment
• Document patient progress clearly and timely to support continuity of care
• Collaborate with physicians, nurses, and other therapists to coordinate patient-centered care
• Educate patients and families on home programs, safety, and fall prevention
• Maintain compliance with facility policies and participate in competency assessments

Qualifications

• Active Physical Therapist license (license verification required)
• BLS certification required
• Weekend availability and willingness to cover both outpatient and inpatient settings
• Must complete Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam
• Clear background and health screenings: drug screen, TB testing/chest x-ray, COVID-19 vaccination, MMR, Hepatitis B, Influenza, Varicella, Tdap, fit test and any required health statements

Knowledge & Skills

• Strong knowledge of outpatient and inpatient rehabilitation principles and modalities
• Skilled in patient assessment, therapeutic exercise, manual techniques, and mobility training
• Proficient documentation and electronic health record skills
• Excellent communication and interdisciplinary teamwork
• Ability to manage caseloads, prioritize tasks, and deliver efficient, compassionate care
