Roles & Responsibilities

• Provide skilled occupational therapy assessment and individualized treatment for residents in a skilled nursing facility
• Develop, implement, and update therapy plans of care that promote function, independence, and safe return to daily activities
• Document evaluations, progress notes, and discharge summaries in accordance with CRG policies and facility standards
• Collaborate with nurses, therapy colleagues, and interdisciplinary teams to coordinate care and optimize outcomes
• Maintain clinical productivity expectations (90%) while delivering high-quality, person-centered care
• Adhere to facility dress code and on-site presence requirements and follow all CRG policies and procedures

Qualifications

• Current Occupational Therapist credential (OTR) or equivalent; eligible for New Mexico licensure preferred
• 1-3 years of clinical experience, ideally with skilled nursing facility (SNF) populations
• Comfortable with on-site, hands-on therapy and a focus on functional, goal-directed care
• Familiarity with documentation standards and productivity expectations in contract therapy settings
• Ability to follow facility dress code (black scrubs) and be present in building during scheduled hours
• Strong communication skills and a team-oriented mindset

Education

• Degree in Occupational Therapy (BS, MS, or OTD) or equivalent clinical credential

Knowledge & Skills

• Skilled in OT assessment and intervention for older adults and rehabilitation populations
• Knowledge of SNF best practices, restorative programs, and discharge planning
• Proficient in clinical documentation and meeting productivity targets
• Ability to design meaningful therapeutic activities that address function and independence
• Effective interdisciplinary communication and teamwork
• Comfortable learning and following facility-specific policies and procedures
