Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document thoroughly
• Monitor and record vital signs, reporting changes in patient condition promptly
• Collaborate with RNs, therapists, and care teams to deliver coordinated care plans
• Perform wound care, ADL assistance, and support with admissions and discharges
• Educate residents and families about care plans and promote a safe environment
• Maintain clear, timely charting in the patient record to ensure continuity of care

Qualifications

• Current LPN license (Licensed Practical Nurse) — PA licensure required or eligible
• Current BLS/CPR certification (or willing to obtain upon hire)
• Comfortable working in a skilled nursing/long-term care environment
• Excellent communication and teamwork skills; dependable and compassionate
• Preferred: prior experience in long-term care or skilled nursing settings

Education

• Graduate of an accredited Practical Nursing program (or equivalent)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Clinical assessment and basic nursing procedures
• Familiarity with wound care and ADL assistance
• Strong charting skills and ability to use electronic health records
• Effective communication with residents, families, and interdisciplinary teams
• Ability to prioritize tasks in a busy clinical setting
