Roles & Responsibilities

Provide compassionate direct patient care and assist with daily living activities
Administer medications and treatments accurately and document care thoroughly
Monitor patient status and report changes to the care team promptly
Collaborate with nurses, therapists, and support staff to create a person-centered plan
Support admissions, discharges, and transfers with professionalism and attention to detail
Maintain a safe, clean environment and follow infection control and facility policies

Qualifications

Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure preferred
Graduate of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willingness to obtain
Ability to pass background check and drug screening as required by the facility
Strong communication skills and a compassionate approach to patient care

Education

Graduate of an accredited Practical Nursing program (LPN)
High school diploma or GED required

Knowledge & Skills

Medication administration and accurate charting
Vital signs assessment and basic clinical monitoring
Familiarity with long-term care / skilled nursing workflows
Effective time management and ability to prioritize patient needs
Teamwork, empathy, and clear communication with patients and families
