Roles & Responsibilities

• Administer medications and perform treatments within LPN scope, ensuring patient safety
• Assess patients and report changes to the Registered Nurse for timely care adjustments
• Accurately document observations, interventions, and resident responses in the medical record
• Participate in team conferences and contribute to individualized nursing care plans
• Provide patient and family teaching to support understanding and continuity of care
• Assist with or initiate emergency measures when sudden adverse developments occur
• Maintain nursing standards and uphold patients' rights in the long term care setting
• Collaborate with ancillary staff to deliver coordinated, high-quality rural healthcare

Qualifications

• Active Michigan LPN license (LARA and/or Nursys must show Active status) - required
• Minimum 2+ years of Long Term Care (LTC) experience - required
• Active American Heart Association BLS certification - required
• Completed Skills Checklist (current to 1 year) pertinent to LTC - required document
• Two supervisory reference forms completed within the last two years - required
• Willingness to comply with facility submission rules (scheduling requests must be provided at time of submission)
• Candidates must provide permanent address; facility will not consider local candidates within a 100-mile radius

Education

• Graduate of an accredited LPN/Licensed Practical Nurse program or equivalent completion required

Knowledge & Skills

• Medication administration and LPN-level clinical treatments
• Patient assessment and ongoing monitoring in a long term care environment
• Thorough documentation skills and familiarity with Epic electronic medical records
• Care plan implementation and ability to follow RN-established plans
• Emergency response and situational awareness
• Patient and family teaching with clear, compassionate communication
• Team collaboration and professional interpersonal skills
• Understanding of long term care resident rights and regulatory standards
