Roles & Responsibilities

• Perform thorough patient evaluations and create individualized treatment plans
• Provide targeted physical therapy interventions to improve strength, mobility, and function
• Educate patients and families on therapeutic exercises, safe movement, and home modifications
• Collaborate with nurses, occupational therapists, physicians, and the interdisciplinary team to coordinate care
• Monitor patient progress, adjust treatment plans, and document outcomes clearly and promptly
• Ensure care plans meet regulatory standards and facility best practices

Qualifications

• Bachelor, Master, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license (applicable to the state you will practice in, e.g., California for this role)
• Experience in a skilled nursing facility or similar post-acute setting preferred
• Strong documentation and progress-note skills
• Excellent communication, interpersonal, and problem-solving abilities
• Ability to work independently and as part of a collaborative team with a compassionate, patient-focused approach

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy
