Roles & Responsibilities

• Provide compassionate nursing care and assist patients with activities of daily living
• Administer medications and treatments safely and document per facility policy
• Monitor and record vital signs, reporting changes in patient condition promptly
• Collaborate with RNs, physicians, and interdisciplinary team members for excellent patient outcomes
• Maintain accurate and timely documentation in the patient chart
• Support infection control, safety protocols, and a clean therapeutic environment
• Educate patients and families about care plans, medications, and follow-up needs

Qualifications

• Current LPN licensure in New Hampshire (required)
• Graduation from an accredited practical nursing program
• Current Basic Life Support (BLS/CPR) certification or willingness to obtain promptly
• Clinical experience in a relevant care setting is preferred but new grads with strong training are welcome
• Strong communication skills and a team-focused attitude
• Ability to work the posted day shift hours and adapt to per-diem scheduling

Education

• Completion of an accredited Practical Nursing (LPN) program or equivalent diploma/certificate

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Skilled in patient assessment and identifying changes in condition
• Comfortable working with electronic medical records and charting accurately
• Good time management and ability to prioritize patient needs
• Strong interpersonal skills for patient and family education and teamwork
