Roles & Responsibilities

• Provide compassionate direct patient care while supporting daily living activities
• Assist nursing staff with vital signs, mobility support, and personal hygiene
• Communicate changes in patient condition clearly and promptly to the care team
• Maintain accurate documentation and support care plans with attention to detail
• Create a comfortable, respectful environment that promotes dignity for residents
• Participate in team huddles and help maintain a clean, safe patient environment

Qualifications

• Current Licensed Nursing Assistant (LNA) certification (VT or NH eligibility noted)
• Active state registry where required and ability to work in the Lebanon area
• CPR/BLS certification preferred or willingness to obtain
• Comfortable working independently on per diem shifts and collaborating with the care team
• Professional attitude, strong communication skills, and a patient-first mindset
• Previous direct care experience preferred but enthusiastic new LNAs are encouraged to apply

Education

• High school diploma or equivalent preferred
• Completion of an approved LNA/CNA program and active certification

Knowledge & Skills

• Strong hands-on patient care skills and familiarity with ADLs
• Ability to safely assist with transfers and mobility support
• Basic knowledge of infection control and safety protocols
• Good observation skills and ability to report changes accurately
• Time management and the ability to prioritize patient needs
• Teamwork and a positive attitude in a fast-paced care setting
