Roles & Responsibilities

• Conduct comprehensive psychosocial assessments for individuals, families, and communities to identify behavioral health needs and strengths.
• Develop individualized, culturally sensitive treatment plans using evidence-based interventions to address depression, anxiety, trauma, substance use, and adjustment disorders.
• Deliver therapeutic services across the lifespan, applying best-practice modalities and adapting approaches for rural and underserved populations.
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues to ensure coordinated, whole-person care and effective treatment planning.
• Provide case management and connect patients with community resources, referrals, and follow-up supports to improve outcomes.
• Maintain timely, accurate clinical documentation and participate in quality improvement initiatives to enhance service delivery.
• Advocate for patients and contribute to outreach efforts that improve access to behavioral health services in a rural healthcare environment.

Qualifications

• Master's degree in Social Work from an accredited program (required).
• Active LISW license in Iowa (LISW required).
• Minimum of 2 years of clinical social work experience preferred; experience with behavioral health presentations such as depression, anxiety, trauma, and substance use.
• Experience working in rural or medically underserved settings is a plus and highly valued.
• Strong interpersonal, organizational, and clinical documentation skills.
• Commitment to serving underserved populations and working collaboratively within integrated care teams.

Education
