Contract Type: Travel Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day). Shift times may vary and could be 7:00 AM - 3:30 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Weekends: every other weekend required (or 4 weekend days in a 4-week schedule). 5x8 day shifts; floats within case management services as needed.

Roles & Responsibilities

• Coordinate patient care across the continuum by leading discharge planning and care transitions
• Perform utilization review and case management using established guidelines to ensure appropriate care
• Document thoroughly in Epic to maintain accurate, audit-ready patient records
• Collaborate with interdisciplinary teams (therapy, pharmacy, social work, IV teams) to create and implement plans of care
• Assess patient needs, order DME, and arrange follow-up resources to support safe discharges
• Apply Milliman guidelines and CMS/HIPAA standards where applicable to support regulatory compliance
• Communicate effectively with patients, families, and payers to advocate for timely, quality care

Qualifications

• AK RN license in hand required for offer (screening for pending licenses noted; auto-offer requires active AK license)
• Minimum 1 year RN experience (acute care/case management experience required)
• BLS certification required
• Epic EMR experience required (ability to navigate hardstops a plus)
• ACM-RN certification preferred
• Familiarity with utilization review and care coordination workflows
• Willingness to float within case management services and work scheduled weekends as required
• Candidates must have permanent address at least 50 miles from facility (locals not accepted)

Knowledge & Skills

• Strong knowledge of case management and utilization review processes
• Discharge planning and care coordination expertise
• Proficiency with Epic charting and electronic documentation
• Familiarity with Milliman guidelines and CMS regulatory requirements
• Comfort working with diverse patient populations (neonates through geriatrics) and specialties like pediatrics, women’s services, ICU
• Solid communication, interdisciplinary collaboration, and care planning skills
• Competence with Microsoft Office and Teams for documentation and team communication
