Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents as an LPN, following care plans and facility protocols
• Administer medications and document responses accurately and timely
• Perform assessments, monitor vital signs, and escalate changes in condition to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to deliver coordinated care
• Support residents with daily living activities while promoting dignity and independence
• Maintain clear, professional documentation and contribute to quality improvement efforts

Qualifications

• Active LPN license in Pennsylvania (or compact/reciprocal licensing where applicable)
• Current BLS/CPR certification
• Experience in long-term care, skilled nursing, or similar settings preferred
• Reliable, professional, and comfortable working evening shifts and every-other-weekend coverage
• Good communication skills and the ability to work collaboratively within a care team

Education

• Graduate of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Strong medication administration and documentation skills
• Familiarity with nursing assessments and care planning
• Knowledge of infection control and resident safety practices
• Ability to prioritize tasks in a fast-paced environment
• Empathy, patience, and strong interpersonal skills for resident-centered care
