Roles & Responsibilities
- Conduct thorough patient evaluations, Start of Care (SOC) visits, reassessments, recertifications, and discharge visits
- Develop and implement individualized occupational therapy plans of care that drive functional goals and independence
- Deliver skilled interventions including ADL/IADL training, upper-extremity strengthening, fine motor work, adaptive equipment training, and energy conservation
- Provide clear, compassionate education to patients and caregivers on home programs, adaptive techniques, and fall-prevention strategies
- Document all clinical care accurately and promptly in Homecare Homebase (HCHB) to meet Medicare and agency standards
- Communicate patient progress, safety concerns, and changes in condition to the interdisciplinary care team
- Collaborate with RNs, PTs, STs, OTAs (when applicable), and leadership to ensure coordinated, high-quality home care
- Provide clinical oversight of OTAs when assigned and permitted under Colorado law and agency policy
Qualifications
- Current, unrestricted Colorado Occupational Therapist license (required)
- Graduation from an accredited Occupational Therapy program
- Valid driver’s license and reliable transportation for in-home visits
- Ability to practice independently in a home-based clinical setting
- Proficiency with, or willingness to learn, Homecare Homebase (HCHB) for documentation
- Comfort working with geriatric and medically complex populations (preferred)
- Familiarity with Medicare home health regulations and documentation (preferred)
- Experience supervising OTAs when applicable (preferred)
Education
- Graduation from an accredited Occupational Therapy program (OT degree)
Knowledge & Skills
- Skilled in ADL and IADL assessments and training
- Knowledge of adaptive equipment selection and training
- Strong clinical documentation skills and familiarity with Medicare Conditions of Participation
- Excellent communication and interdisciplinary collaboration skills
- Ability to assess home safety and implement fall-prevention strategies
- Time management and independent clinical decision-making in community-based settings
