Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the Lebanon center
- Administer medications and treatments safely and according to policy
- Observe and monitor patient condition, reporting changes to the care team
- Accurately document care, assessments, and interventions in the medical record
- Collaborate with nurses, providers, and support staff to coordinate patient care
- Assist with admissions, discharges, and transfers to ensure smooth transitions
- Educate patients and families about care plans and post-discharge instructions
Qualifications
- Current LPN license in New Hampshire (required)
- Clinical experience as an LPN preferred; new grads welcome to apply for per diem roles
- Basic Life Support (BLS) certification or willingness to obtain
- Strong communication skills and ability to work collaboratively in a team
- Reliable, compassionate, and patient-focused approach to care
Education
- Graduate of an accredited practical nursing (LPN) program or equivalent
Knowledge & Skills
- Solid knowledge of practical nursing principles and patient care techniques
- Skill in medication administration and monitoring for side effects
- Competence with documentation and charting best practices
- Ability to prioritize tasks in a fast-paced clinical environment
- Comfort working with interdisciplinary teams and following care plans
