Roles & Responsibilities
- Evaluate patients’ mobility, strength and functional needs and develop personalized treatment plans
- Provide skilled therapeutic interventions to improve movement, reduce pain and restore independence
- Document patient progress and outcomes clearly and timely in the electronic health record
- Collaborate with nurses, physicians and rehab team members to coordinate care and discharge planning
- Educate patients and families on home exercise programs and injury prevention strategies
- Participate in team meetings and contribute to quality improvement initiatives
Qualifications
- Valid Physical Therapist license to practice in California (required)
- Current BLS/CPR certification
- Experience in orthopedic, outpatient, or post-acute rehab settings is preferred
- Strong clinical assessment, manual therapy and therapeutic exercise skills
- Ability to complete facility background check and required credentialing in a timely manner
- Excellent communication skills and a collaborative approach to patient-centered care
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
Knowledge & Skills
- Thorough knowledge of physical therapy evaluation and treatment techniques
- Skilled in therapeutic exercise, gait training, manual therapy and mobility aids
- Proficient documentation and use of electronic health records
- Understanding of discharge planning, safety assessment and patient education best practices
- Ability to work within an interdisciplinary team and adapt to varied patient populations
