Shift Type: Monday-Friday, 8-hour day shifts; start time between 6:00 AM and 10:00 AM (consistent schedule required), Day shift
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and wounds, then develop and implement effective plans of care
- Use standardized assessments and evidence-based practice to guide clinical interventions
- Administer manual techniques, ambulation training, therapeutic exercises, modalities and wound care as indicated
- Assess patient progress, adjust treatments, and evaluate outcomes to ensure effective care
- Document all regulatory and clinically required information accurately in the electronic documentation system
- Record treatment minutes precisely per regulatory requirements and company policy
- Provide counseling and education to patients, families, caregivers and other providers to support recovery and independence
- Participate in patient care conferences, utilization review, rehabilitation meetings, trainings and in‑services
- Obtain necessary DME and supplies and supervise/support PTAs, rehabilitation aides and students
Qualifications
- Missouri State License required (Compact accepted)
- Minimum 1 year of PT experience required
- New grads OK with Level II experience (Level I observation alone is not sufficient)
- LTC or Rehab experience required
- Ability to supervise PTAs and mentor students or aides
- Familiarity with regulatory and clinical documentation requirements in long‑term care settings
Knowledge & Skills
- Physical therapy assessment and treatment planning
- Wound care management in a rehabilitation/LTC setting
- Use of standardized assessments and evidence‑based practice
- Manual techniques, ambulation training, therapeutic exercise and modalities
- Electronic documentation systems and accurate treatment minutes recording
- Understanding of DME needs and safe use of assistive devices
- Knowledge of APTA guidance and the Missouri State Practice Act
- Strong communication and interdisciplinary collaboration skills
