Roles & Responsibilities

• Provide compassionate bedside nursing care to residents in a long-term/rehab setting
• Administer medications and treatments accurately and document per policy
• Monitor and record vital signs, reporting changes in condition promptly
• Support residents with activities of daily living while promoting independence
• Collaborate with the interdisciplinary team to develop and follow care plans
• Maintain accurate, timely charting in the resident record
• Ensure a safe, clean environment and adhere to infection prevention protocols
• Advocate for residents' needs and communicate effectively with families

Qualifications

• Current LPN license (Licensed Practical Nurse) in Massachusetts or eligible for state licensure
• Basic Life Support (BLS) / CPR certification required or obtained upon hire
• Experience in long-term care, skilled nursing, or rehabilitation preferred
• Strong interpersonal skills and the ability to work effectively on a care team
• Reliable, professional, and committed to resident-focused care

Education

• Graduate of an accredited LPN program
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation accuracy
• Resident assessment and recognizing changes in condition
• Familiarity with electronic medical records and charting best practices
• Infection control, safe patient handling, and falls prevention
• Clear communication with residents, families, and clinical team members
• Time management and prioritization in a fast-paced clinical environment
